Use this form to request recognition for a couple celebrating their Diamond Wedding anniversary or an anniversary every 5 years thereafter.

This form can also be used to recognise an individual celebrating their 100th Birthday and every year thereafter.

We prefer as much notice as possible to ensure we can accommodate requests.

For Anniversaries we will need the following information:

  • Name
  • Maiden name (if applicable)
  • Address
  • Date of marriage
  • Town of marriage

For Birthdays we will need the following information:

  • Name
  • Maiden name (if applicable)
  • Address
  • Date of birth
  • Town of birth
