Renew a Disabled National Entitlement Card

Use this form if:

  • Your Disabled National Entitlement Card (NEC) is about to expire or has already expired.

Don’t use this form if your card is:

  • Lost
  • Stolen
  • Broken

Instead, use the National entitlement card replacement form.

Before You Start

To fill in this form, you need:

  • A Disabled NEC card that is expired or expiring soon.
  • To live in Fife.
  • To prove you still qualify for the card.

You might need to:

  • Upload a photo of a benefit letter or other proof.
  • Or give us details we can check.

Uploading Your Documents

To help us with your application, we need to see some documents from you.

You can upload them to the form. We’ll only keep them as long as we need to.

How to Upload

You can:

Tips
