Book your appointment to register a death
This form is for booking a telephone or in-person appointment with a Registrar to register a death.
You will need the following:
- Medical Certificate of Cause of Death (Form 11) issued by a doctor or hospital or the serial number
You will be asked for the following documents, if relevant:
- Deceased person's birth certificate
- Their marriage certificate(s)
- Their medical card
Have the following documents if you want to use the Tell Us Once Service:
- Deceased person's National Insurance number (if available)
- Their passport or passport number (if applicable)
- Their driving licence or driving licence number (if applicable)
- Surviving spouse's National Insurance number (if applicable)
- Their Blue Badge (if applicable)
- Details of public sector pension (if applicable)
