Book your appointment to register a death

This form is for booking a telephone or in-person appointment with a Registrar to register a death.

To book a death you will need the following:

  • Medical Certificate of Cause of Death (Form 11) issued by a doctor or hospital or the serial number

If you can please have the following documents:

  • Deceased person's birth certificate (if available)
  • Their marriage certificate(s) (if applicable)
  • Their medical card (if available)

If you wish to use Tell Us Once Service, please have the following documents:

  • Deceased person's National Insurance number (if available)
  • Their passport or passport number (if applicable)
  • Their driving licence or driving licence number (if applicable)
  • Surviving spouse's National Insurance number (if applicable)
  • Their Blue Badge (if applicable)
  • Details of public sector pension (if applicable)
