This form is for booking a telephone or in-person appointment with a Registrar to register the birth of your baby.

Call 03451 550077 if you don't have a birth card, or you want to register twins, triplets, or more.

What you will need to book your appointment:

  • Birth card or A4 letter sheet from maternity hospital or midwife.
  • Parents' birth certificates (if available)
  • Marriage certificate (if applicable)
