Apply for a National Entitlement card (NEC) for starting secondary school
All pupils moving from Primary 7 into Secondary school will be automatically opted in for Young Scot/PASS on their NEC/MyFife card.
Use this form to select the type of NEC/MyFife card and give consent.
The form will ask for the following:
- The current primary school
- The intended secondary school
- A recent passport-style photo of the pupil, no larger than 5MB
You'll be asked to sign in or register for a mygov.scot myaccount, in order to complete this form. This account is also used for paying for school meals, access council tax, and many other council services.
